Founded by Shannon Wilcken, a healthcare leader with more than 25 years of healthcare experience and a certified Senior Transition Specialist (STS), BridgeCare combines healthcare knowledge, compassionate guidance and practical transition support to help families make informed decisions with greater confidence.
